EmblemHealth, one of the leading Group Providers for NY Metropolitan area.  EmblemHealth has physcians and hospitals throughout NY State.  Below are benefits and rates for groups with only one employee on the payroll.

The Plan we are marketing to Sole Propriorters is an HSA high deductible plan.  The deductible is $5800 per person and for a family the annual deductible is $11,600.  This means that an individual must have $5800 is medical bills before Emblem covers the cost at 100% or $11,600 for family coverage.  This does pertain to In-Network: Routine Physicals, Screenings, Immunizations and Well Baby Care visits - all of these are covered at 100%.

You can also set up an HSA account at no cost through Melon Bank which is sponsored by EmblemHealth.  The maximum contribution for an HSA in 2010 is $5800.  There are catch up provisions, but please see IRS HSA rules for specific details.

Tax Documents Required for 1 person group plans require 2 of the following tax documents and document should have companies name on it.  Forms you  can submit are:

Schedule C; Form 1120-S; Form 1065 with a Schedule K1; CT-4-S; Schedule F; NYS-45 or NYS-45-ATT; Article of Inc.;  Signed copy of most recent Schedule SE; Signed copy of the full tax return for the most recent tax year accompanied by the appropriate W2s.
