Can your client answer "NO" to each of these
questions:

Is the proposed
insured currently hospitalized, confined to a nursing home, hospice or
bed, institutionalized, receiving home health care, waiting for an organ
transplant or, within the last 12 months, received kidney dialysis?

Has the proposed
insured ever been diagnosed or treated by a member of the medical
profession for an Immune Deficiency Disorder, Acquired Immune Deficiency
Syndrome (AIDS) or AIDS Related Complex (ARC?

Has the proposed
insured been diagnosed as having a terminal medical condition that is
expected to result in death within the next 12 months?

Prior to age 25, has
the proposed insured had, or been diagnosed with or received treatment
for mental retardation, cerebral palsy, muscular dystrophy, spina
bifida, cystic fibrosis or un-operated heart defects?
If your client can
truthfully answer "NO" to each of the above questions they qualify for a
Graded Benefit Policy with an Ultimate Face Amount of up to $15,000.
Can your client answer "NO" to
each of these questions?

Has the proposed
insured ever been diagnosed with congestive heart failure, Alzheimer's
disease, dementia, kidney failure or Lou Gehrig's disease?

During the past 24
months has the proposed been diagnosed as having: a stroke, heart
attack, aneurysm, enlarged heart, angina, pacemaker implant or any
procedure to improve the circulation to the heart or brain?
-
During the last 36
months, has the proposed insured had, been diagnosed or received
treatment (including taking medication) for:
A. Any form of cancer (other than
basal cell skin cancer?
B. Emphysema, chronic obstructive
pulmonary disease (COPD), black lung disease or any
other
chronic respiratory disorder, excluding asthma?
C. Kidney disease, kidney failure,
liver disease, drug or alcohol avuse?
D. Multiple Sclerosis, Parkinson's
Disease, or psychosis?

During the last 24
months has the proposed insured experienced complications of diabetes,
including: insulin shock, diabetic coma, amputation, eye or kidney
disorder or diabetes not
under control with current medication?
If your client can
truthfully answer "NO" to each of the above questions they qualify for a
Policy with Full Immediate Benefits with a Face Amount of up to $25,000.
