must be appointed and licensed with the Carrier
BEFORE you may solicit business for that carrier.
Please download the forms for any carrier you wish to represent.
Please complete and sign the required
forms wherever indicated.
Please FAX the completed, signed
forms back to us as soon as possible together with a copy of your License.
Where E
and O Insurance is required please be sure to FAX evidence of your E & O
coverage to us to us as well.
(E&O) These carriers
require you to have at least $1,000,000 of Errors and Omissions insurance,
otherwise they will not appoint you
